EVALUATION QUESTIONNAIRE OF THE WAY CONDUCTING CLASESS BY AN ACADEMIC TEACHER
The questionnaire can be completed:
- for 21 days before the end of the classes. The last day to complete the questionnaire is the last day of the classes with the relevant teacher (tu można wpisać konkretne terminy, w zależności od tego jak zajęcia są zdefiniowane w systemie USOS)
- when you log in on your individual account:
- on the website usosweb.usos.pw.edu.pl;
- through the Mobile USOS PW app on Android or iOS devices.
The aim of the questionnaire is to collect opinions on the ways of conducting classes and to provide a mechanism of influencing the improvement of the quality of teaching in a given field of study. The results of the questionnaire are part of the evaluation of how academic teachers complete their teaching assignments. The questionnaire is ANONYMOUS.*
We ask you to complete the questionnaire during the class, at a time specified by the academic teacher.
Your opinion about classes is very important for us.
We kindly ask you to provide sincere, well-thought-out and objective answers to the questions.

1) on the website of USOSweb: usosweb.usos.pw.edu.pl,
2) through the Mobile USOS PW app.
*the questionnaire is anonymous because the USOSweb system does not save any information linking the user with his/her responses. Linking the responses with a particular student is not possible even for the IT administrator. The USOS system stores only information about the time of completion of the questionnaire and the course group so that multiple evaluations of the same course cannot to be submitted by the same student. Academic teachers have direct access to average evaluation results for the courses they teach.
